One of the most common mistakes in AS evaluation is relying too heavily on a single parameter. A complete echocardiographic assessment should integrate:
Peak Velocity & Mean Gradient Continuous wave Doppler remains essential in evaluating AS severity. Typical severe AS findings include:
Understanding the Continuity Equation The continuity equation is one of the most important calculations used in AS evaluation. This calculation estimates the aortic valve area by comparing flow through the LVOT to flow across the stenotic valve. But here’s the challenge: Small errors in LVOT diameter measurement can dramatically affect the final AVA result because the LVOT radius is squared in the calculation. That’s why understanding proper measurement technique is critical. The Importance of the Dimensionless Index When LVOT measurements are difficult or suboptimal, the dimensionless index (DI) can provide additional clinical insight. A DI <0.25 is typically consistent with severe AS.
This parameter helps improve confidence in severity classification and reduce dependence on potentially inaccurate LVOT measurements. Low-Flow, Low-Gradient AS: One of the Biggest Challenges in EchoNot all severe AS presents with high gradients. Some patients demonstrate:
